Transitions

Adapting to the changes that accompany a life-limiting illness can be difficult, frustrating and sometimes confusing. It can be a challenge to keep one’s independence while also learning to accept help from others.

Transitions is a program that can help empower individuals going through this difficult time to make choices that affect the quality of their lives.

The Transitions Program offers ongoing case management, while a staff of trained volunteers offers clients a variety of services, including:

  • Transportation
  • Companionship
  • Help with errands & shopping
  • Respite care for family members & caregivers
  • Help with household chores & food preparation

Individuals who have been diagnosed with a life-limiting illness and have a life expectancy of two years or less are eligible for the Transitions Program. Clients may continue to seek curative medical treatment. If you or someone you know...

  • needs help understanding his or her illness
  • is feeling isolated or lonely
  • is having difficulty doing light housework or shopping for food
  • or can use some support...

This service is free to clients and their families thanks to volunteer services and support from the Blue Envelope Health Fund, the Daniel's Fund and the Wyoming Community Foundation.
