Information About Hospice Care

What is Hospice?
Considered to be the model for quality, compassionate care for people facing a life-limiting illness or injury, hospice and palliative care involve a team-oriented approach to expert medical care, pain management, and emotional and spiritual support, expressly tailored to the patient's needs and wishes. Support is provided to the patient’s loved ones as well. The focus of hospice relies on the belief that each of us has the right to die pain-free, with dignity, and that our loved ones will receive the necessary support to allow us to do so.

Our Services

Outpatient Care
Hospice focuses on caring, not curing and, in most cases, care is provided in the patient's home. Members of the hospice team visit the patient’s home, providing assistance, compassion and support, allowing the patient to remain in the comfort of his or her own home, surrounded by people who care. Outpatient care is also given to residents of long-term care facilities and assisted living facilities.

Inpatient Care
For various reasons, many patients are unable to remain in their own home. For those, CWHP offers inpatient care in the Charles M. & Cara Lou Chapman Hospice Home. There, patients receive 24-hour skilled nursing care in a comfortable, home-like environment. Meals are provided in the dining room or in the comfort of the patient’s room. The kitchen, living room, children’s play area and reading room are always open to patients, family and visitors.

The Hospice Team

CWHP cares for a patient’s mind, body and spirit through a collaboration of professional staff and volunteers, including our staff of nurses and nursing assistants, a medical director, social worker, bereavement counselor, volunteer coordinator and chaplain.

Who is Eligible for Hospice Care?
Hospice care is for any person who has a life-threatening or terminal illness. Most reimbursement sources require a prognosis of six months or less if the illness runs its normal course. Patients with cancer and non-cancer illnesses are eligible to receive hospice care.
